The Good Shepherd Clinic - Basic eligibility criteria The patients served at the Good Shepherd Community Clinic must meet the following criteria: Adults over the age of 18 regardless of gender, race, creed, religion, nationality, or sexual preference Income level not to exceed 150% of the Poverty Guidelines for the current year established by the United States Department of Health and Human Services No health insurance Resident of Morgan County Documentation required: Proof of income such as previous pay stubs, previous year tax statement, bank statements, and completion of financial data on intake
